We also provide safe housing in our Supported Independent Living homes in western Sydney, as well as Plan Management and Support Coordination services to NDIS participants.

So what will you be doing in this amazing role?
- Manage client care needs
- Complete initial assessments of clients using your clinical nursing skills to determine their home care needs
- Provide client progress reports to relevant stakeholders, such as internal staff, families, carers and external service providers
- Review and monitor client care notes to ensure services are being delivered effectively and consistently
- Identify trends in client care
- Facilitate a variety of clinical training to our community support workers
What are you going to bring along with you?
- It would be great if you had tertiary qualification in Nursing or a related discipline
- Previous experience in a related role, such as client or case management
- Ability to problem solve and work effectively in a very fast paced environment with multiple competing priorities
- Current and active driver’s licence
