Client Coordinators work closely with our clients and Workforce Coordinators to support our client's needs.
This is a full-time office role that involves managing the enquiries and requests of our clients and working within clients' care plans and funding.
Workforce Coordinators organise and coordinate the deployment of carers in the community.
They collaborate with our Client Coordinators and Afea Carers to ensure consistent and high-quality service delivery to our clients.
Our Care Managers manage our high-care clients at every stage, from initial assessment to reviews.
As part of their client case management, they recommend appropriate services in accordance with the client's care plan and work in conjunction with the client and the client's care team, including their support workers, Disability Client Coordinators, Support Coordinators, Plan Managers, caregivers and other relevant parties.
They also build long-term, trusted relationships with industry alliances in the aged care and disability industries on behalf of Afea.
