We are seeking a passionate Care Coordinator to join our experienced services team. This role will report directly to a Team Leader and will see you responsible for organising, scheduling and managing the deployment of carers as well as taking a proactive, coordinated and collaborative approach to ensure consistent and quality service delivery to clients.

In this role, you will find yourself:

  • Coordinating and actioning client service requests and booking in line with Afea’s policy and guidelines.
  • Managing feedback, complaints and incidents from both clients and carers.
  • Monitoring both funding and renewals of the client.
  • Allocating, coordinating and deploying rosters in line with SCHADS Award and client budgets.
  • Maintaining regular contact to build strong relationships with clients, carers, families and other stake holder involved.
  • Managing calls and emails in a timely and effective manner.

We are looking for a candidate who possesses the following:

  • Previous experience in a related role preferably in disability and aged care sectors.
  • Qualifications in Health, Disability, Aged or Community Care.
  • Excellent communication and relationship building skills.
  • Able to problem solve and work effectively in a very fast paced environment with multiple competing priorities.
  • Knowledge of National Disability Insurance Scheme is preferred.
