Due to our rapid growth and expansion, we are seeking a passionate Support Coordinator to join our experienced support coordination team. Reporting directly to our Support Coordination Manager you will support clients in connecting with service providers and informal supports. You will also find yourself on the road visiting clients face to face and helping them to understand their NDIS plan and related funding.

In this role, you will find yourself:

  • Empowering clients with a voice and choice of plan implementation that helps them meet their NDIS plan goals.
  • Connecting clients with service providers and informal supports.
  • Monitoring & evaluating services provided to clients ensuring they are working towards client’s goals.
  • Working collaboratively with clients, families, providers & other stakeholders involved.
  • Attending community and networking events.

We are looking for a candidate who possesses the following:

  • Tertiary qualifications in Social Work or Disability Services.
  • Experience in case management.
  • Previous experience within a human services environment (preferably within the Community, Disability and/or Aged Care sectors)
  • Current and active driver’s licence
